Insurance Coverage

We require our patients to have insurance coverage for appointments with our providers. As such, we need to have up-to-date insurance information on file PRIOR to your appointment. This includes the insurance policy number, plan name, and if required by your insurance Dr. Subroto Bhattacharya listed as the PCP (not Danvers Family Doctors, Jennifer Richardson, or Hayley Mateen) regardless of what provider you typically see.
It is the patients responsibility to be in contact with their insurance provider to ensure that their policy is up-to-date with the correct provider listed, to confirm in-network coverage, the exact plan and tier, as well as any co-pay or deductible information pertaining to their plan.
Because we service so many patients through numerous insurance companies, we do not have the capacity to contact insurances regarding each patient’s plan.
Patients are also expected to pay their copay at the time of the visit. If there is a question as to the eligibility of your insurance, we may require you to pay $100 up front and sign a form stating that you are responsible for the appointment balance if the insurance does not cover if the patient does not choose to reschedule.
We do our best to cover as many patients as we are able, however, we are only able to contract with so many. As such, it is possible that we do not take your insurance or that changes will be made such that we will no longer take your plan. At this time (July 2025), we take the insurance packages listed below, but they are subject to change over time.
