New Patients

Danvers Family Doctors is currently accepting new patients of all ages to the practice with appointments available with all three of our providers.
To register yourself as a patient and schedule an appointment please call the office at 978.762.6262.
Please note the following items that need to be completed before you can be scheduled for ANY appointments.
1. Patient Registration
General demographic information relayed to one of our front desk staff over the phone.
2 . Insurance Validation
First ensurance that your insurance carrier is one that is taken by our office by visiting the insurance section of the home page. Approved insurances can be provided to front desk staff at the time of registration. It is the patients responsibility to ensure that Dr. Bhattacharya is listed as the primary care provider on file, should your insurance require this information as well, as well as to submit an image of your card on the patient portal or bring the card with you to your first appointment.
3. Prior Medical Records
In order to provide the best continuation of care, our office requires some form of previous medical records. Most important are vaccination records and documentation for any medications you will continue to need prescribed or health conditions that may need to be monitored. These records can be faxed to our office at 978.750.8312. Many practices will require a medical release form to be signed to show your consent to release records to our practice. If they do not provide their own forms, forms can be found on our website’s homepage under medical forms.
